inconsistent php form into sql results

I'm building a blog type site and I made a php/html form to load posts into my sql database. Sometimes it works, and sometimes it doesn't. It consistently uploads the images to my root images folder but the posts table isn't updated about 95% of the time. My code seems to be fine.
Why is this happening?

Hello proxette,

Other than the TEXT data type having a 64kb limit, there should be no reason the mysql server would not allow the data to be inserted into the table. I can step through the process to watch both the code and the server for you, however I would need to know the URL of the testing page so I can trace the code for other issues along with test data that you know to fail. You can send that information to docs@webhostinghub.com and we can take a look from there. As we do not monitor that email address, be sure to place a comment here after sending the information so we know it has been sent.

The server adds nothing to any files, so if you do not add the DOCTYPE line, it will not insert a default one into the file.
