Embed form on website

I cannot get the HTML email sign up form (embed) from Mail Chimp to work. I'm using iweb. The box shows up on the website, but does not have any words or does anything when clicked on. Just a long white box. Any suggestions?

Hello,

I checked your site out but cannot tell which page you are referring to. I assumed the Contact page would be the one but when checking the source code there is no code for a form there.
