In the Premium Web Builder, you can insert a Feedback Form that creates fields that visitors can fill out and submit electronically. You specify the information that you are requesting from your visitors (such as name, phone number, email address, etc.) to ensure that you receive the information that you are looking to collect. There is also a "comments" field which gives you visitors a place to give their own comments.
First, log into your Premium Web Builder and navigate to the Edit page. Click on the page in the Site Map that you want to add the Feedback Form to, and then drag the Feedback Module to the place on the page that you want the Feedback Form to be:
Once you have placed the form you will not see the questions in many browsers, but just the bar. On the Feedback Form bar, click on Add/Edit Fields:
The following screen will appear, which will allow you to chose what fields your visitor will fill out on the form. Under Name will be the name seen by your visitors, and Type specifies the type of response your visitors can leave. For example, "String" means a string of text that the visitor can enter, while E-mail means they will enter an email address. Once you have put in all the fields you would like, click "OK."
The form may still not show in the Web Builder depending on the browser you are using, however you can click on the "Preview" button at the bottom of the screen to see what the form will look like on your website:
If you like how the form looks, Publish your website to make it appear live on your website. If not, you can make more changes to the form in the Properties or Add/Edit Fields.
